If You Want to Have Stock Electronically Transferred

From Your Investment Advisor

Stock may be transferred to an existing endowment or to open a new endowment without a physical certificate by requesting that your investment manager/advisor initiate the contribution. First, send the attached Contribution Notification Form to The Catholic Foundation. This will indicate your intent to make a gift. Second, your investment advisor should be instructed to contact Tonya Kirkland at Fifth Third Bank, (812) 456-3883 prior to an electronic transfer. Please note: if the Trust & Investment Management Division is not notified prior to the transfer, your gift may be rejected for lack of proper identification.
